Amlodipine Vs. hydrochlorothiazide: is there any disparity in its antihypertensive effect or not?

چکیده
We have read the article "Comparison of effects between calcium channel blocker and diuretics in combination with angiotensin II receptor blocker on 24-h central blood pressure and vascular hemodynamic parameters in hypertensive patients: study design for a multicenter, double-blinded, active controlled, phase 4, randomized trial" by GC Oh., et al. and found it quite interesting to learn the testing of effectiveness between amlodipine and hydrochlorothiazide in combination with losartan. After 4 weeks of therapy, non-responders were exposed for dose titration of losartan/amlodipine 100/5 mg daily or losartan/hydrochlorothiazide 100/25 mg daily. However, authors didn't increase in the amlodipine dose from 5 to 10 mg from randomization. However, as per literature both drugs are non-significant in their blood pressure lowering effect. The clarification on above point will further allay the efficacy concerns of clinicians and result in wider usage of future published data.

Randomized study to compare valsartan ± HCTZ versus amlodipine ± HCTZ strategies to maximize blood pressure control
OBJECTIVE Delays in achieving blood pressure (BP) control may increase morbidity and mortality in patients with hypertension. Thus, deciding which antihypertensive agent to use and at what dosage, in addition to determining when to initiate combination therapy and which agents to combine, is important for achieving BP control. METHODS This randomized, double-blind, 14-week study was conducted...
متن کاملLong-Term Electrolyte Effects during Initiation of Antihypertensive Therapy with Amlodipine or Hydrochlorothiazide in Diabetic Nigerians
Information is scarce regarding the effects of amlodipine or hydrochlorothiazide therapy on serum and urine electrolyte profiles in hypertensive Nigerians with type 2 diabetes mellitus. Therefore, to evaluate whether amlodipine or hydrochlorothiazide would be preferable to initiate treatment, we randomized 40 newly diagnosed hypertensive subjects with controlled type 2 diabetes mellitus aged 43...
متن کاملPrior Medications and the Cardiovascular Benefits From Combination Angiotensin‐Converting Enzyme Inhibition Plus Calcium Channel Blockade Among High‐Risk Hypertensive Patients
BACKGROUND The ACCOMPLISH (Avoiding Cardiovascular Events Through Combination Therapy in Patients Living with Systolic Hypertension) trial demonstrated that combination therapy using amlodipine, rather than hydrochlorothiazide, in conjunction with benazepril provided greater cardiovascular risk reduction among high-risk hypertensive patients. Few trials have evaluated the effect of prior antihy...
متن کاملMonotherapy with amlodipine or hydrochlorothiazide in patients with mild to moderate hypertension: Comparison of their efficacy and effects on electrolytes
BACKGROUND Amlodipine and hydrochlorothiazide (HCTZ) are commonly prescribed in Nigeria either as a monotherapy or in combination with other drugs. The present study was designed to investigate the antihypertensive efficacy of monotherapy with amlodipine or HCTZ and their effects on electrolyte profile in patients with mild to moderate hypertension. METHODS A single-blind randomized clinical ...
متن کاملRole of valsartan, amlodipine and hydrochlorothiazide fixed combination in blood pressure control: an update
The treatment of moderate or severe hypertension in most cases requires the contemporaneous use of multiple antihypertensive agents. The most available two-drug combinations have an agent that addresses renin secretion and another one that is statistically more effective in renin-independent hypertension. The practice of combining agents that counteract different mechanisms is the most likely e...
